It plugs into a yellow connector near the cam sensor.

The 2 nuts holding it on are 12mm.

O2 sensor does not use bolts. There are studs threaded into the O2 sensor housing (turbine elbow) and the O2 is held on by the 2 12mm nuts. And no, a new O2 does not come with any new hardware, just a new gasket.

You better be careful taking those nuts off, if you snap the studs you will be up shits creek without a paddle. In other words, you will need to take the downpipe off and remove the turbine elbow and get a new one or have a shop remove the snapped studs.

Try using a penetrating oil like PB or like he said use heat, a propane torch should do the trick.
